What is IVF PLUS?

IVF PLUS is a new modality of in vitro fertilization that consists of transferring embryos that have previously been genetically diagnosed. A study of the embryo’s chromosomes is carried out and only those with the capacity to implant are transferred.
The main reason that not all patients who undergo IVF treatment become pregnant the first time is that the transferred embryos have genetic alterations that nature blocks.
In the following graph you can see how fertility decreases with age and abortions increase. This is due to chromosomal abnormalities in the embryos. When we transfer embryos from eggs from young donors to women over 40, pregnancy rates are once again high and abortion rates are low.

When is IVF PLUS indicated?

An IVF PLUS is indicated in patients who have a higher probability of having embryos with alterations. Among them we can highlight:

Women over 35 years
Women with previous implantation failures
Women with low ovarian reserve
Women with a history of miscarriage
Presence of a severe male factor such as very low sperm count or genetic alterations in the sperm.

How is IVF PLUS performed?

To perform IVF PLUS, it is necessary to follow the following steps:

IVF requires ovarian stimulation to retrieve several eggs and have a better chance of success. This phase lasts about 10-12 days, during which 3 or 4 ultrasound and hormonal controls must be performed to adjust the dose based on the response obtained. In this phase in the ovaries the follicles are growing. Each follicle contains an egg floating in the so-called follicular fluid.

Once the stimulation is finished and when the gynecologist considers that the follicles have matured, the follicular fluids are extracted. Fluids are aspirated by vaginal ultrasound. This process is performed under sedation, requires fasting and lasts about 10-15 minutes. It does not require subsequent admission, on the same day you can return home.

The same day that the eggs are retrieved, the man must deliver a semen sample that will be processed by the laboratory. The sperm with better mobility are recovered.

It consists of fertilizing the eggs in the laboratory with the prepared semen. At our center we always perform the variant with microinjection (ICSI) of a motile and normal spermatozoon in each mature egg, to ensure a higher percentage of fertilization. With this technique, the number of spermatozoa and their mobility become a secondary factor.

The embryos are cultured in vitro for 5 days, until the blastocyst stage. During this phase, morphological and dynamic aspects (cell division rhythm) are monitored, which allows us to select the evolutionary embryos.

On the fifth day of culture, a group of cells is extracted from each embryo for genetic analysis. All biopsied embryos are frozen pending the results of the genetic analysis. Freezing is done using the vitrification technique. The result is obtained after 15-20 days. If there are normal embryos, the woman’s endometrial preparation cycle is scheduled to receive the selected embryo or embryos.

The patient, advised by the medical-biological team, decides which embryo or embryos to devitrify and transfer. The embryologist loads the selected embryo(s) into a cannula and they are transferred to the uterus with the help of an abdominal ultrasound to control the placement site. For the transfer it is not necessary to come fasting. It lasts about 5-10 minutes and after 30 minutes of rest you can go home.

10-11 days after the transfer, an analysis called beta-HCG is performed to verify the success of the procedure.
