In order to get pregnant, both male and female reproductive systems have to work correctly, without any physical, chemical or immunological obstacles.

Once the egg is fertilized and turned into an embryo, it has to go through the Fallopian tubes to the mucus of the uterus, or endometrium, which should be prepared for the implantation and the following development of the embryo. It is at this point when the pregnancy occurs.

Situations that can hinder this process:
  • Ovarian factor, or anomalies in the menstrual cycle, as ovaries that don't work due to natural causes or that have been useless or removed because of any disease.
    • Menopause (premature or not): the oocyte quality has to do with the age of the woman. That is to say, the age shows the chances of pregnancy.
    • Endometriosis: disease that affects the quality of the oocyte, among other things.
    • Polycystic ovary (when the ovulation does not develop properly) or hyperprolactinaemia (oversecretion of prolactin) are other examples of the ovarian factor.
  • Fallopian tubes blockage: also known as tubal obstruction, can involve fluid (hydrosalpinx) or not. It is often inflammatory. It can be overcome thanks to in vitro fertilisation technique.
  • Cervical factor: the cervix (or cervix uteri) shows hostile environment, with thick mucus, acids or inflammatory substance for the sperm. The likely solutions are the intrauterine insemination or the in vitro fertilisation.
  • Uterine factor: it is generally caused by an immature or undeveloped endometrium, by internal tumours (as polyps or submucosal myomas), or due to uterine malformations that in many cases aren't appreciable in routine gynaecological examinations.
